Application for PAN through Online Services One single character initial is allowed in middle name field in PAN application for individual category. (Please note that name in proof of identity and proof of address should exactly match with the name in application). • New PAN Card application form is only for the individuals,firms, body corporate and companies applying for the pan number first time. • PAN Number onceallottedis permanent for that individual / company and having two PAN numbers is a offence. • if you have lost the PAN card or after applying has received the PAN number but Card was not received then apply for re-issue of pan card. • If information is to be updated in Income Tax database, like change of address or email address or mobile number, then apply for change / correction application form. • If you do not have current address proof or proper identity proof then Gazzeted officer certificate can be attached in the given format above along with the copy of Gazzeted officer identity card.
